Long ago I had a Paladin, a Charger, called Junior_SayOw (For Junior Seau the NFL linebacker of the Chargers, and now the Dolphins)
My favorite one was from a friend: "Osis_OfTheLiver" Before LoD, when he killed Diablo, he became "Sir Osis_OfTheLiver" I imagine there were other sir jokes, like Loin_Steak becoming Sir Loin_Steak.
Another friend of mine had an Amazon called Of_Virginity. In HC, she achieved "Guardian Of_Virginity" but he stopped playing a long while back.
I had a character named of_TheFlies who never got to Lord of_TheFlies because he expired. The fact that he was on Europe did not help.

Got boring after 30 or so chars. Now I come up with whatever seem fun at the time:

Right now I'm going to be playing FreezerBvrn for my holy freeze ladder paladin. Bvrn of course since the correctly spelled version was already taken.

Knew a guy who named his hardcore D2 classic necromancer "Chocula" so that when he beat Diablo in nightmare, he'd be "Count Chocula."
