Seeing as how I just managed to hit 71, I suppose there's no loss in sharing this information.
While with certain addative synergies, it is quite clear what skill distribution is optimal (just chose the bigger one till 20), but things get interesting when you have two or more factors multiplying from different skills.
With the hammerdin, there are three: conc multiplyer (halved 60,15% a level = base 30, 7.5%), vigor and blessed aim synergies (addative at base 0,14% a level), and BH itself, which, naturally, should be maxed early (the actual, complex numbers show the ideal space to to somewhere like 7 points in the other skills when BH is maxed; of course, spend points acourding to your comfort with the mana cost).
Since the concentration & synergy bonuses are linear, the computation is relatively simple (the synergies are bundled together since they're addative; done in excel):

Since the synergies add more per level and have no base bonus, it makes sense that the actual pumping of Conc should be delayed.
Taking + to skills into account is surprizingly simple. + to skills adds to to Conc, but not to synergies. So, simply folow the chart as though you had spent the points in Conc to bring it up to it's + level, and as though you hadn't on the synergies.
