That macro should do everything you'd want, whether the target is targetting an NPC or a friendly player, or not.
I had always used
#showtooltip Righteous defense
/cast [noharm,nodead][target=targettarget,noharm,nodead] Righteous Defense
anyway. The nodead is of course not specifically needed. Though it's good to know that the defualt was changed anyway and that my macro should still be fine and dandy.
