KingOfPain took one to Hell Baal at about 66.
She hired a rogue, and equipped every slot except for armor, helm, weapon. Lots of passives, not sure what items, but I think Crushing Blow was involved. Yep, she was a tank for her rogue. :) You can find his write up at RBD.
