I've heard a couple of things, but the one that makes the most sense (and I would consider a bug) is that her pet had some effect on it, when it was unsummoned. When she summoned it again, the effect was still active, and then activated (which provided the kaboom). Now, she knew it would happen, and chose the AH to summon her pet just to kill people, so the griefing aspect remains, but I'd think that an effect staying on an unsummoned pet would be a bug of sorts. But that's just me.

There is a 'Time Bomb' like curse that a boss in Molten Core can cast. Not having been in MC I don't know the particulars, but I believe it is something like "After x seconds curse causes AoE damage of xxxx to friendlies". Generally this is not an issue, as you have mages assigned to removing the curse, and the amount of time is in the 20-30 seconds range, so there is generally plenty of time to get the curse off if the raid party has any brains.
Curses and de-buffs not being removed by dismissing pets was a purposeful decision, as a Warlock with the right talents can re-summon in 0.5 seconds (once per 15 minutes.) So if your pet got a bad curse or other negative effect, you could potentially dismiss and re-summon instantly without the negative effect on your pet.
