Due to the size of labels.
"Cap" is a very small label, so it doesn't take up much room on the screen, but "Godly Plate of the Whale" is much bigger than "Cap," and if there are a lot of 'big' labels on screen ("Scroll of Town Portal" is the most common one.) then you haven't got a hope in hell of seeing all the labels.
Just pick up a few items until the one you want appears, then pick it up and lose the crap.
