I was under the assumption the drops were not pre-determined at level/map generation, well, at least from experience with the PSX version.
When using emulator savestates or the Diablo PSX in-game load/save option, when you kill a monster (Lazarus for example), you can exploit him to drop a specific item, or unique too. I'm guessing this does not hold true for the PC version, hmm...
