I very rarely fall off -- but I DO fall off sometimes. The more you use grip, the faster you seem to lose it. I've lost 50+ grip in a heartbeat.
The main requirement of it is to keep a cool head and use your abilities as soon as they are available, but without spamming them.

Hearing that it can be gotten at level 40 is making me very happy. <3 My paladin alt is definitely getting it now.
